Medical Questionnaire
Due to improving safety standards in the State of Hawaii, we are now requiring all divers to complete a medical questionnaire before any dive trip. This is our way to verify that no guest has any health contradictions that may prevent them from safely diving with us and other guests. Some health contradictions may require approval by a physician before diving. Please review the form above to check if you will require a physician’s approval.
You will be required to answer these questions as part of your waiver before your trip.
